hum.....

what type of monitor do you have attached to your graphic card ?
how much shared memory have you configured for your graphic card in your BIOS 
?

It seems that X doesn't see the capabilites of your graphic card.... In my 
XFree.log, mode detection section is something like that:
Mode: 34 (1024x768)
        ModeAttributes: 0x9b
        WinAAttributes: 0x7
        WinBAttributes: 0x0
        WinGranularity: 64
        WinSize: 64
        WinASegment: 0xa000
        WinBSegment: 0x0
        WinFuncPtr: 0xc00059a3
        BytesPerScanline: 1024
        XResolution: 1024
        YResolution: 768
        XCharSize: 8
        YCharSize: 16
        NumberOfPlanes: 1
        BitsPerPixel: 8
        NumberOfBanks: 1
         etc...........

In your log file, you have only zeros in this section which is bad......
Do you have recently plug a new PCI card on your system ?
