You do realise that:
you are specifying display modes in your screen display subsection which don't fit with those of
your monitor...
In your monitor section you are giving a specific synch instead of a synch range which is probably
why you are getting:
(II) S3VIRGE(0): Not using default mode "1024x768" (hsync out of range)

I would
1. remove whatever you have as far as modelines in your monitor section (the defaults should work)
2. add the mode 1024x768 in your screen display subsection -you need only keep the one for depth
16 since this is your default depth - you can also restrict yourself to one resolution to simplify
your life, 
I suspect this might already get you going if not re-post mainly your config file...
