Ok, I've worked out what does it: I have focus on mouse over, and if I press ctrl while focus is on something else, the ctrl does not carry over to the wine application. eg, I am using eog to view images, I press ctrl while eog has focus and move the cursor over to the open dialogue of the flickr uploader and then click. All previous files are deslected. Is this something that could get fixed?
