Are you using ISO-image? Maybe it is just damaged? I'm installing my game from legal CD, then replace only one file - FlatOut2.exe, and add d3dx9_30.dll directly into the game folder (when launching form clean prefix). All works fine. Which way you use to start the game? File browser? Have you tried to launch it from terminal?
