It did occur to me that the HDD could have died.
I've seen many instances where this has happened during an OS install.

The clue is that the machine runs perfectly from a live CD but fails 
to write to the HDD.

I use a USB caddy to check HDD's externally.  In most cases the disc 
is not recognised, though I've had some that are but the HDD has been 
write locked. ie Read Only.  In these cases they can sometimes be 
unlocked using the manufacturers utility.
