[RFC PATCH 01/24] target/iblock: remove an extra argument

The twp arguments to the functions op and opf which are REQ_OP_XXX and
REQ_XXX flags are belongs to the two different namespaces, i.e. they
can be combined safely given that REQ_OP_XXX takes 8bits of the flags
and rest is available to REQ_XXX flags.

Replace op and op_flag arguments with opf.
