Fixed this up for the 'Mutual CHAP authentication' section to reflect
the proper *_mutual naming for TPG endpoint + NodeACL authentication.

Also, the discovery authentication uses the mutual_* naming convention
in rtslib for some reason, but the underlying configfs attributes are
all still using *_mutual.
