Ok, I'm mentioning this because the Intel + Open-FCoE folks have spent
quite a bit of time this year testing tcm_fc w/ active I/O shutdown post
conversion to target_submit_cmd() + se_cmd->cmd_kref accounting.

IIRC this conversion did go in for v3.2-rc5 code, so I'm thinking there
is some else in >= 3.3.0 that is missing here..
