[PATCH 0/4] cleanup conditional ordered tag, reservations and ALUA code

This series removes various indirections and flags in the ordered tag,
reservation and ALUA code.  All these can in practive only have a mode
where they are off (for pscsi) or where they have the full SPC-3 features,
but there is some dead or almost dead code allowing SCSI-2 only modes
on virtual devices, and generally some superflous indirections.
