Issue in RESERVE6 command

Hi List,

After writing some initial testcases for RESERVE6 I have found an
issue with RESERVE6 in LIO.
I use LIO on Fedora17.

If an initiator takes out a RESERVE6 on a LUN and then logs out
without releasing the reservation, then any other existing sessions
as well as new sessions are failed with RESERVATION CONFLICT.

Even if trying to re-login using the same initiator name, the LUN is
now permanently unavailable.

To test/reproduce, use my iscsi test tool at :
https://github.com/sahlberg/libiscsi.git

To run the test I use :
./bin/iscsi-test --test="*0422*"
iscsi://10.9.0.2/iqn.2003-01.org.linux-iscsi.fedora17.x8664:sn.84052e857f74/0

Run it twice to see that the second login now fails.

The test tool uses these two initiators names you need to allow access for :
  iqn.2007-10.com.github:sahlberg:libiscsi:iscsi-test
  iqn.2007-10.com.github:sahlberg:libiscsi:iscsi-test-2
