Initially I wanted to do lazy allocation as well and use the presence
or absence of a lun (or device, for the second patch) as replacement
for TRANSPORT_LUN_STATUS_ACTIVE/TRANSPORT_LUN_STATUS_FREE.  But not
all code would be straight-forward to change and I ran out of time.

So sorry for these half-arsed patches.  They fix real bugs for me, but
I really should have done more.
