Hey Madhu,

After some light testing this afternoon, these cleanups appear to be
working as expected.  I don't personally have strong feelings on the
individual naming changes, and am more concerned about what makes Andrew
@ Qlogic happy.  :)

Also, a followup patch to remove typedef usage following Christoph's
recommendation for kernel coding style would be useful.
