[PATCH V2] KVM: SEV: Acquire vcpu mutex when updating VMSA

Adds vcpu mutex guard to the VMSA updating code. Refactors out
__sev_launch_update_vmsa() function to deal with per vCPU parts
of sev_launch_update_vmsa().
