Patch "loop: fix crash if blk_alloc_queue fails" has been added to the 3.4-stable tree

This is a note to let you know that I've just added the patch titled

    loop: fix crash if blk_alloc_queue fails

to the 3.4-stable tree which can be found at:

The filename of the patch is:
     loop-fix-crash-if-blk_alloc_queue-fails.patch
and it can be found in the queue-3.4 subdirectory.

If you, or anyone else, feels it should not be added to the stable tree,

loop: fix crash if blk_alloc_queue fails

If blk_alloc_queue fails, loop_add cleans up, but it doesn't clean up the
identifier allocated with idr_alloc. That causes crash on module unload in
idr_for_each(&loop_index_idr, &loop_exit_cb, NULL); where we attempt to
remove non-existed device with that id.
