Patch "blk-mq: Create hctx for each present CPU" has been added to the 4.12-stable tree

This is a note to let you know that I've just added the patch titled

    blk-mq: Create hctx for each present CPU

to the 4.12-stable tree which can be found at:

The filename of the patch is:
     blk-mq-create-hctx-for-each-present-cpu.patch
and it can be found in the queue-4.12 subdirectory.

Currently we only create hctx for online CPUs, which can lead to a lot
of churn due to frequent soft offline / online operations.  Instead
allocate one for each present CPU to avoid this and dramatically simplify
the code.
