Techinically this fix might be backported to 3.18 & 4.1 too, because
.bi_vcnt is still used in req_gap_to_prev() before commit 5e7c4274a
("block: Check for gaps on front and back merges").
Also you need to pick up the 1st three patches for the fix if you want to
do that.
