This patch depends on commit 3ef28e83ab15 "block: generic
request_queue reference counting"
upstream which is nominally too big for -stable (7 files changed, 102
insertions(+), 75 deletions(-)).

However, if you're open to taking that larger patch I'll reflow this
one.  It fixes a class of crash signatures by re-purposing an existing
percpu reference count.
