This error can be reproduced simply by clicking "Add to Address Book "
while reading a message. The right window becomes blank and the error
above is logged in the error_log.

The addressbook entry is never made. I also tried removing the
existing addressbook, in case it was somehow corrupt.

The URL associated with the link that produces the error is:

https://mail.example.com/webmail/plugins/add_address/add_addresses.php?mailbox=INBOX&passed_id=7&view_as_html=&startMessage=1&passed_ent_id=0&account=

I would think the address to be added should be a part of the URL?

I haven't made any changes from the default config_default.php file
other than copying it to config.php.

I've also sent in a separate email the output from phpinfo() on the
server as well as my local config.php. Hope that's okay.
