First stop scanning your outgoing email with mailscanner or make sure that
it does not tag emails and does not put useless ads.

Then check your SquirrelMail configuration. If attachment directory is not
set to /tmp, test your PHP configuration
(http://www.squirrelmail.org/wiki/TestPHPSettings). It does not matter what
you have in php.ini, when you forget to apply it or have overrides in other
locations.
