First thing to check is the difference between accounts (or how they are being used) that have those folders and ones that don't. Perhaps the script that created the bulk accounts created folders at the same time and the other script did not, leaving it to the user or the user's mail client to do so. SquirrelMail tries to create those folders if they don't exist, depending on this setting in the configuration tool: 3. Folder Defaults ==> 16.
