Check your apache configuration. maybe clients are accessing different
virtual hosts or default virtual host and some host has broken <directory>
access list. You must make sure that access to your squirrelmail directory
is allowed globally or in all virtual hosts that use it. If you set some
Alias or dedicated virtual host for webmail, they must have corresponding
<directory /some/path/to/squirrelmail> configuration with correct options
and access restrictions.

If you use some security filters in apache, make sure that you get same
error, when these modules are disabled.
