Change Passwd Plugin

I have a box running
squirrelmail-1.4.8-5.el5.centos.3

The change_passwd plugin shows a blank screen.
Change Passwd
4.2-1.2.8

Compatibility
2.0.14

When I enable the debugger plugin I get the following once logging in.

Notice: A session had already been started - ignoring session_start() in
/usr/share/squirrelmail/functions/global.php on line 434
