Yahoo, AOL, and Hotmail in particular are notorious for junking mail from small, low-traffic servers. If I had to guess, I'd say the SquirrelMail headers put your message over the junk threshold when combined with other factors, such as: mismatching HELO or rDNS, short DNS TTL, dynamic IP, etc. Not that SquirrelMail is often used to send spam so much as spam often contains forged SquirrelMail headers. Although SquirrelMail may be a factor, it's most likely not the culprit.
