You need

access to attrs=userPassword
by dn="replace with administrator's DN" write
by anonymous auth
by self write
by * none

Other ACLs and plugin's configuration depend on 'allow bind_v2' and 'access to
*' restrictions in slapd.conf. Some OpenLDAP versions might require write access
right to shadowLastChange attribute.
Please note that in change_ldappass configuration ldap attributes should be
written in lowercase.
