If you have FAM libraries on imap server, do you have FAM service running? If IMAP service and webmail are on same machine, are you sure that -R option is used in tcpserver command? Are you sure that 'localhost' DNS name is resolved correctly. wiki instructions are not accurate. You don't have to create daemontools startup scripts, qmail-ldap provides startup scripts in /var/qmail/boot directory. You must use --enable-unicode parameter without arguments when you compile courier-imap. If you limit unicode support to iso-8859-1 and utf8, you break SquirrelMail server side sorting in some translations. Are you sure that ADS does not throttle connections coming from one address? Evolution and Outlook can cause one ldap lookup during IMAP connection. SquirrelMail does 3-4 IMAP connections during login. More connections are created when you use interface.
