I had perhaps the same problem a couple of days ago. I fianlly figured out that 2 things were happening unexpectedly. First, the vacation program defaults to only respond to the same address once a week. So unless you turn off vacation for a user and then turn it back on, it will not send a reply to the same address. man vacation tells you how to change this setting. Second, I had a procmail script that scanned email using Spamassassin and deleted emails that were scored as very likely spam. One of my custom rules was pushing the vacation response over this threshold and so the response was being deleted before reaching the user's mailbox. I had been relying heavily on the ALL_TRUSTED rule as a sort of whitelist to determine if emails were legit, but because vacation does not deliver via SMTP ALL_TRUSTED was not being triggered.
