Nigel, I had experienced the same problem a few days ago and I posted my issue here. I discovered I had two issues: 1. did not patch the change_passwd plugin 2. I was using a higher version of compatibility plugin. I had to downgrade to version 1.3 as per the advise I got from google. after doing the above two things, my change_passwd plugin worked.
