You can use any username because it is ignored.
Please, check your config once more.
Is it possible that you (indirectly) deny access to cache manager before you allow
it, thus it's always refused?
Don't you have line that denies access from the machine you try to access
before config lines like these?
# grep \^http_access /etc/squid/squid.conf
http_access deny !Safe_ports
http_access deny CONNECT !SSL_ports
http_access allow localhost manager
http_access allow manager <machine>
http_access deny manager
http_access deny to_localhost
http_access allow <...>
http_access deny all
