AFAICT, Squid does not have code that can customize headers of a regular 200 (Connection established) response to a CONNECT requests (with or without MitM). This functionality is rarely needed (because most clients tend to ignore most CONNECT response headers), but quality pull requests adding that functionality are welcomed.
