it turned out it's the basic_ldap_auth helper which produces this output.

it turned out that I don't have to worry. I'm now able to reproduce it easily: the log lines occur every time when a user is sending an invalid password.
Fortunately that does not happen too often and I don't have to care anyway. It's just an annoyance to have it all logged in cache.log.
