I assume that the URL in question matches the sites_server_1 ACL. Please note that IP-based URLs may not match that ACL, especially if the reverse DNS lookup does not work the way you would expect it to work. If the URL in question matches and setting "nonhierarchical_direct off" does not help, please see Amos response.
