Regarding the question about LDAP I cannot answer since I am not familiar with this helper but I am sure others are. I noticed that you are using squid 3.4.0.3 which is basically a beta version.
You should consider using a newer version such as stable 3.4.5.
There are many bug-fixes since the version you are using.

The problem is I have a users scattered throughout AD. They don’t all reside in ou=baz. Some are in ou=qux.

We are using Squid Cache: Version 3.4.0.3 ascertained from the official squid repo. We are on CentOS 6 x86_64.
