Hey Peter,
Regarding the question about LDAP I cannot answer since I am not
familiar with this helper but I am sure others are.
I noticed that you are using squid 3.4.0.3 which is basically a beta
version.
You should consider using a newer version such as stable 3.4.5.
There are many bug-fixes since the version you are using.
Take a look at:

Hello Everyone,
Why does /usr/lib64/squid/basic_ldap_auth require an ou in the -b argument?
If I give it a:
-b “dc=foo,dc=bar”, it doesn’t work The binary itself fails.
But when I do a:
-b “ou=baz,dc=foo,dc=bar”
it works.
The problem is I have a users scattered throughout AD. They don’t all reside in ou=baz. Some are in ou=qux.
We are using Squid Cache: Version 3.4.0.3 ascertained from the official squid repo. We are on CentOS 6 x86_64.
