Basically the idea is nice!
I would say that there is a overhead which slows down browsing that can slowdown loaded systems. Also instead of responding always with a OK and store-id=URL in a case of a "non modified storeID" use the "ERR" that will lower the bandwidth usage and will also reduce squid re-processing of unneeded bits\bytes.

You need to mention that there is a readme file that show the needed settings in squid or to include this readme file inside the distributed tgz file.

The idea of this helper using base64 encoding was used in couple other helpers in the past and which I found very nice.
