If you are using multiple workers, please see
http://wiki.squid-cache.org/Features/CacheManager#SMP_considerations

If you are not using multiple workers, the corresponding table entry at
the above URL may still contain useful information for you.

If I interpret the code correctly, Squid is using the source IP address
of the client connection rather than the XFF header value. Please note
that ICP clients may also be counted.
