The stats from <access.log> indicate a 2.2% hit rate. Is this typical?

Only if you are grepping the logs for "HIT". The REFRESH_UNMODIFIED are known as near-HIT and count as HIT in the metrics.

Your manager report shows a 6 - 90% HIT rate and 1-25% bytes savings.
