Ideas?  Should I be looking at squid, or bind for the issue?  Has anyone
seen anything similar before?  All other proxy and dns operations work

what do you get in the access log?
what is the cache.log output when you start squid instance?
what version of squid are you using? ("squid -v" output)

Access log shows a miss for the page, as expected.
