I would suggest you to read this:
http://wiki.squid-cache.org/ConfigExamples/Authenticate/Ntlm

not just because you need to read but since there are options you don't use which makes it a bit weird.
like:
auth_param ntlm use_ntlm_negotiate on

It should work for you.

also a nice reference is here:
http://www.cyberciti.biz/faq/squid-ntlm-authentication-configuration-howto/

Which I really like.

Hello,

i got 2 Machines

1 - Working (Squid 3.1.2 with latest Dansguardian and NTLM)

2 - new (fresh install Debian7 + Squid 3.3.5 with latest Dansguardian and
NTLM Auth against a Windows Server 2003)

I did the same Installation like the Working one, but still get the
Authetification Window in my Browser when i try to use the NEW one.
Samba, Winbind, Kerberos working well get all information with Klist, wbinfo
-u or -g and get success with
wbinfo --authenticate Admin

Before i forget, at the moment im useing the Proxy Settings with Port 3128
