Several strong signs there that you are using a Squid-2 series release. Does this still ocur with the currently supported 3.1 release?

More details needed. What is this slow request? how fast are your DNS servers responding?

If it is taking 45 minutes, how do you know its the first request? did you calculate the start times from logged ending time+duration?
