It is for reverse-proxy with delay pools active unless care is taken to omit the site(s) from the pools.

Do delay pools only apply to squid-backend connections?

yes. "delay_pools" delay pools do.

Or is there a way to make it apply to client-squid connections?

"client_delay_pools" delay pools. which is available in 3.2 beta.

OR the QoS controls which are available in slightly variant forms with all Squid releases since 2.6.
