to fit this exactly, although it looks like setting "default" to the
failover instance would do the trick. However, the documentation
indicates that ICP is needed for those options and these are Apaches
rather than Squids.

The order of cache_peer configuration does this type of failover by default when no selection method is given. The peers are listed top-down in order of preference. The first available origin is used.
