Where i have to open port 123 ? On a Modem-router adsl and i must to have nat to the proxy external address ?

The Windows XP systems require a NTP server that they can connect to in order to time sync. You can either open TCP/123 and UDP/123 for your clients to the internet, or set up NTPD on your squid server and specify it as your NTP server.
