Hi!

This could come a little "off-topic", but: which VPN are you using? if
you happen to be using OpenVPN, try adding the option "persist-tun",
also, there is a chance that you can make squid actually restart when
the VPN goes down and up again (by using the "down" and "up" options,
that calls down and up scripts).

Anyway, please keep us informed!
All of those are already done.

I've reset the cache. rm -rf /var/cache/squid/* and let it rebuild it. The cache seemed the only difference between this installation and two
other ones with the same configs that remain stable...  If that doesn't
work, I may need to replace the server.

Have you changed your never_direct as per http://wiki.squid-cache.org/SquidFaq/ConfiguringSquid#How_do_I_configure_Squid_to_work_behind_a_firewall.3F
