Amos, I think you are right.

I replaced all error messages, restarted squid and yet IE7 was still flashing the (now nonexistant) error message. It happens when you fail proxy authentication (by clicking cancel) and getting CACHE_ACCESS_DENIED. If there is 302 later on, it flashes it before showing the page. It doesnt show it on 200's. I suspect it it cached in system (Vista), closing/opening IE7 does not help.
 
It does not happen with Firefox on RedHat.
 
It happens with older squids too now that I tested.
 
Oh well, that is one entire day and half of my life that I won't get back!
