The follow_x_forwarded_for allows a parent cache to read the X-Forwarded-For header for logging (and possibly for ACLs). As long as you don't disable X-Forwarded-For (http://www.squid-cache.org/Doc/config/forwarded_for/ or http://www.squid-cache.org/Doc/config/header_access/), the header will be populated by Squid.
