I use something like this:

auth_param basic program /usr/lib/squid/ldap_auth
  -b "ou=Users,dc=mydomain,dc=com"
  -D 'cn=binddn,dc=mydomain,dc=com"' -w 'pass'
  -v 3
  -H ldap://ldap1.mydomain.com ldap://ldap2.mydomain.com
