Memory size = 2 Gigs
cache_mem = 256 M
disk cache = 16 Gigs
I ran memtest86 on the memory but in a different box. It checked out OK.

well, what's your memory size, what are your cache_disk and cache_mem sizes?
did you run memtest6 on the machine?
