Thanks a lot Amos,

I'm sure this problem will be fixed in STABLE15.

I edited the src/HttpHeader.cc directly and changed the lines:

{"Max-Forwards", HDR_MAX_FORWARDS, ftInt},

to become

{"Max-Forwards", HDR_MAX_FORWARDS, ftInt64},

Seems it works now. Waiting the latest version STABLE15

I'm just rolling STABLE15 now. It should be available in a few hours. You may prefer to use that release bundle instead.

Otherwise for individual patches, the 3.0 ones are to be found at:
  http://www.squid-cache.org/Versions/v3/3.0/changesets/

The one you are after is labeled "Bug #2635: Incorrect Max-Forwards header type". Either on the first page, or the page titles 3.0.STABLE15 when the rolling is done.
