That config was a bit weird, but there are some entries in your listed config which worry me terribly... The global access to permit Purge opens a number of DDoS vectors. And the use of "always_direct allow all" as the first always_direct line will prevent your otherwise listed cache_peer link every being used. Also the fact that the cache_peer link settings are listed LAST in the config, instead of first indicates its not going to be used even if available.
