Yes. It's not related to the request handling, only to the reply object headers.
2) Is there a way to know when a refresh_pattern is being matched other than the noticing it in the log when the url pattern is seen and squid reacts.
There should be something in cache.log with the right debug_options I think 22,3 looks promising.
