That is normal behavior for NTLM.
ICQ disconnects frequently - it says it is not authorized (in logs).
ICQ may not be picking up the system credentials and sending them to squid. check its configured properly. Or if its not capable of doing that you will need to make an ACL bypass to allow ICQ through unauthenticated.
